Consent Letter for Name Split – Indian Passport Renewal (VFS Submission)
This document is a signed consent letter required for Indian passport renewal applications when the applicant’s current passport lists both the first and last name under the “Given Name” field, and the surname field is blank.
By submitting this letter to VFS Global / the Indian Consulate, the applicant formally authorizes the correction and split of their name into a proper Given Name and Surname in the re-issued passport.
