This downloadable template is designed for parents applying for a Surrender Indian passport Certificate for a minor who automatically acquired U.S. citizenship through their parents and therefore does not possess an individual Naturalization Certificate.

The document provides a properly worded notarizable affidavit to be signed by both parents, explaining:

How and when the minor derived U.S. citizenship
Why is a Naturalization Certificate not available for the child
Confirmation that the parents hold valid Naturalization Certificates

This affidavit is commonly requested by VFS Global / Indian Consulates when a child has a U.S. passport but no separate naturalization certificate.
